ارتباط پرفشاری خون با فشار داخل چشمی در بیماران مراجعه کننده به بیمارستان امیرالمومنین شهرستان رشت در سال 1390

زمینه و هدف

فشار بالای داخل چشم در گذشته اغلب جزء معیارهای تعریف گلوکوم زاویه باز بوده است. در مطالعه حاضر به بررسی ارتباط فشار خون و برخی عوامل زمینه ای اپیدمیولوژیک با فشار داخل چشمی بالا در جمعیت بستری در بیمارستان امیرالمومنین در طول سال 1390 پرداخته شده است.

مواد و روش ها

در این مطالعه توصیفی گذشته نگر 180 بیمار با فشار داخل چشمی بالا وارد مطالعه شدند. داده های جمع آوری شده با استفاده از نرم افزار آماری SPSS v.18 مورد تجزیه و تحلیل قرار گرفت.

یافته ها

زنان 55 درصد بیماران را تشکیل می دادند. میانگین سنی بیماران 88 / 11 ± 64 / 67 سال، میانگین فشار خون سیستولی 7/ 16 ± 180 و دیاستولی 8/ 18 ± 8/ 136 میلی متر جیوه بود. میانگین فشار داخل چشمی راست 35 / 3 ± 16 / 16 و فشار داخل چشمی چپ 76 / 3 ± 49 / 16 بود. رابطه متغیرهای جنس، سابقه فشار خون بالا، مصرف الکل، سابقه چربی خون بالا و سابقه بیماری قلبی با فشار داخل چشمی راست معنی دار شد ( 0/05 > P ). ارتباط معنی داری بین تغییرات فشار خون سیستولی و فشار داخل چشمی ( IOP ) مشاهده شد ( 05 / P = 0 )، درحالیکه این ارتباط در تغییرات فشار خون دیاستولی با IOP مشاهده نشد ( 0/05 < P).

نتیجه گیری

مطالعه حاضر نشان داد که فشار خون سیستولی و فشار خون سیستمیک بطور موثر و مثبت می توانند با فشار داخل چشمی بالا مرتبط باشند. اما این تغییرات تحت تاثیر سن قرار ندارند. جهت دسترسی به اطلاعات بیشتر انجام مطالعه ای بلند مدت جهت ارزیابی شاخص های مطرح شده و پیشرفته تر از جمله تعیین تاثیر ضخامت عدسی بر افزایش فشار داخل چشمی پیشنهاد می گردد.

Relationship of Hypertension with High Ocular Pressure in Patients Referred to Amir’almomenin Hospital of Rasht in 2012

Introduction and Aims

High ocular pressure is often related with open angle glaucoma. In this study we evaluated association of hypertension and some other underlying factors with high Intraocular pressure (IOP) in patients of Amir’almomenin hospital during 2011-2012.

Materials and Methods

This is a descriptive retrospective study. 180 Patients with high IOP hospitalized during 2011-2012 in Amir’almomenin hospital in Rasht were included. Collected data was analyzed by SPSS v.18.

Results

55% of patients were women. Mean age of them was 67.64 ± 11.88. Mean systolic and diastolic blood pressure were 180 ± 16.7 and 136.8 ± 18.8, respectively. Mean right and left IOP were 16.16 ± 3.35 and 16.49 ± 3.76, respectively. There was a significant relation between gender, history of hypertension, alcohol abuse, hyperlipidemia and cardiac disease with high IOP in right eye (P < 0.05) that had a straight relation with high IOP. Also there was a meaningful relation between changes of systolic blood pressure and IOP (P = 0.005) but there was no relation between changes of diastolic pressure and IOP (P > 0.05).

Conclusion

Present study showed that systolic blood pressure and hypertension can effectively and positively be related with high IOP. But these changes were not affected by age. For getting more data, it is recommended to perform a longitudinal study for evaluation of indicated factors of this study and more advanced indexes such as determination of thickness of cornea for better evaluation of relations with high IOP.
