بررسی عوامل خطر ایجادکننده سنگ‌‌های کلیوی: مطالعه مورد-شاهدی

چکیده
مقدمه

سنگ‌‌های ادراری که سنگ کلیه جزئی از آن می‌‌باشد، سومین مشکل شایع دستگاه ادراری محسوب می‌‌شود. با توجه به بار اقتصادی بالای این بیماری و روند رو به افزایش آن شناخت عوامل مرتبط با این بیماری می‌‌تواند موجب پیشگیری از شیوع آن وکاهش هزینه‌‌های درمانی گردد. لذا این مطالعه با هدف تعیین عوامل خطر ایجاد کننده سنگ‌‌های کلیوی انجام شد.

مواد و روش‌‌ها

این مطالعه از نوع گذشته‌‌نگر و به صورت مورد-شاهدی بود که نمونه گروه مورد را 124 نفر افراد بالای 18سال مراجعه کننده به مراکز سنگ شکنی وکلینیک خاتم الانبیاء یزد تشکیل دادند. نمونه گروه شاهد نیز شامل 186 نفر افراد بالای 18 سال بودند که هیچگونه علامت و یا سابقه سنگ کلیه نداشتند و همزمان با گروه مورد در بیمارستان حضور داشتند. اطلاعات از طریق پرسشنامه و به صورت خودگزارش دهی جمع‌‌آوری شد. داده‌‌ها با استفاده از نرم‌‌افزار SPSS v.19 توسط آزمون‌‌های کای دو ، تی مستقل و رگرسیون لجستیک مورد تجزیه و تحلیل قرار گرفت.

یافته‌‌ها

در مطالعه حاضر سطح تحصیلات ((0/77، 0/51)= CI 95%، 0/63=OR) و سابقه خانوادگی ((1/3،3/3)= CI 95%، 2/1=CR) تنها عواملی بودند که ارتباط معنی‌‌داری با ابتلا به سنگ کلیه داشتند. نتایج همچنین نشان داد که مصرف چای کمتر باعث کاهش شانس ابتلا به سنگ می‌‌شود.

نتیجه‌‌گیری

هرچند سابقه خانوادگی و به عبارتی ژنتیک افراد قابل تغییر نیست ولی می توان با بالا بردن سطح آگاهی افراد در خصوص رژیم غذایی درصد مبتلایان را تا حد زیادی کاهش داد.

Risk Factors for the Development of Renal Stones: A Case-Control Study

Background & Aims: Kidney stones are the third most common problems in the urinary tract. According to the economic burden and its rising trend, it seems, knowing the associated factors can prevent the spread and cost of it. Therefore, this study was conducted to determine the risk factors of kidney stones.

Methods: This study was a retrospective case-control study that the case group consisted of 124 persons over the age of 18 which referred to lithotripsy centers and Yazd Khatamolanbiya clinic who for the first time they had been diagnosed with stones and the control group consisted of 186 persons over the age of 18 who had no symptoms or history of kidney stones. They were at the same time attended in the hospital with case group. The data collected through a questionnaire and interviews face to face. The data analyzed using SPSS19 software by chi-square and t logistic regression.

Results: In this study, education level(OR=0.63, CI 95%=(.51,.77)) and family history (OR=2.1, CI 95%=(1.3,3.3)) were the only factors that were influential in the development of kidney stones.The results also showed that drinking water, low-fat dairy, consumption of fruits, and vegetables reduce the risk of stones, but drinking tea has the direct relationship with the risk of kidney stones.

Conclusion: Although family history and genetic expression are unchangeable, but we can greatly decrease the percentage of patients with raising awareness and changing of their diet.
