گزارش یک مورد مدیریت بیماری گلنزمن در جراحی کیست هموراژیک تخمدان

چکیده
مقدمه

بیماری ترومبوآستنی گلنزمن یک بیماری ارثی و اختلال در عملکرد پلاکت است. این بیماری اگرچه نادر است ولی شیوع این بیماری در جوامعی که ازدواج فامیلی در آن‌ها رواج دارد بیش‌تر است. یکی از مشکلات مربوط به این بیماری انجام اعمال جراحی به‌ویژه اعمال بزرگ است که ما در گزارش موردی حاضر مدیریت یک مورد کیست هموراژیک تخمدان مربوط به بیمار مبتلا به بیماری گلنزمن را بیان کردیم.

مورد

بیمار یک دختر نوجوان که با سابقه بیماری گلنزمن با درد در ناحیه شکم به بیمارستان مراجعه کرده است. پس از انجام سونوگرافی و شرح حال و آزمایشات مربوطه کیست هموراژیک تخمدان برای وی تشخیص داده شد برای بیمار سه ویال فاکتور هفت فعال نوترکیب قبل از عمل و در حین عمل نیز هر دو ساعت سه ویال و بعد از عمل هر شش ساعت سه ویال از این فاکتور تا زمان بند آمدن خون‌ریزی و هموستاز دریافت کرد. در نهایت بیمار چهار روز بعد با حال عمومی خوب مرخص گردید.

نتیجه­گیری

با توجه به مطالعات گذشته و تجربه ما نیز توانستیم با تزریق پلاکت قبل عمل و فاکتور هفت در حین عمل و بعد از آن خون ریزی حین و بعد از عمل این بیمار را کنترل کنیم.

چکیده English

Introduction

Glanzmann's thrombasthenia is the hereditary disease of abnormal function in the platelets. Despite being rare, this disease Given the direct relationship between the rate of cousin marriages and the disease prevalence. The main problem in the treatment of the disease is performing particularly major surgeries. In the present case report, we stated case management of the hemorrhagic ovarian cyst with a history of Gelzmen's disease.

Case report

With a history of Gelzmen's disease, the adolescent girl presented with a pain in the abdominal area. After performing ultrasound and taking a medical history and the related tests conducted, the patient was diagnosed with a hemorrhagic ovarian cyst. Before the surgery, the patient received three vials of the recombinant activated factor VII. During and after the operation, she was received three vials in every two hours, and three vials every six hours until homeostasis. Finally, the patient was discharged four days later with the satisfactory general condition

Conclusion

Concerning the previous studies as well as our experience, we could manage the patient bleeding during and after the surgery for treatment of the hemorrhagic ovarian cyst by preoperative injections of the platelets and the factor VII during and after the operation
