Comparison of the Impacts of In-Person and Virtual Education on Self-Efficacy and Treatment Adherence among Patients with Hypertension

Abstract
Introduction: Effective hypertension management requires treatment adherence and increased patient self-efficacy. This study compared the impact of in-person and virtual education on self-efficacy and treatment adherence in patients with hypertension.

Methods: This three-group, pre-and post-clinical trial included 120 hypertensive patients treated at Farshchian Heart Hospital Clinic in Hamadan. Patients were selected through convenience sampling and randomly assigned to in-person education (40 patients), virtual education (40 patients), and control (40 patients) groups using permuted block randomization. The control group received routine clinic care, whereas the in-person education group attended four weekly 40-45 minutes’ sessions. The virtual education group received the same content through virtual platforms. The Mandanlou Treatment Adherence Questionnaire and the Sherer Self-Efficacy Scale were completed before and two months after the intervention.

Results: Before the intervention, there were no significant differences in demographic variables and mean scores of treatment adherence and self-efficacy among the three groups (P > 0.05). However, post-intervention, significant increases in treatment adherence and self-efficacy were observed in both the in-person and virtual education groups compared to the control group (P < 0.05). Additionally, the in-person education group showed a significantly greater increase in treatment adherence and self-efficacy compared to the virtual education group (P < 0.05).

Conclusion: The results indicated that both in-person and virtual education effectively improved treatment adherence and self-efficacy in patients with hypertension. However, in-person education had a greater impact than virtual education; therefore, it is recommended that nurses use in-person and virtual education based on the patient's needs and conditions in addition to traditional methods of patient education.
