درک بی‌ثباتی ارتباطی و هیجانی در بیماران مبتلا به اختلال شخصیت مرزی (یک مطالعه کیفی)

چکیده
مقدمه: یکی از شایع‌ترین، ناتوان‌کننده‌ترین و مهلک‌ترین شرایط روانی، اختلال شخصیت مرزی بوده که با هزینه‌ها و پیامدهای جدی همراه است. پژوهش حاضر با هدف شناسایی و درک بی‌ثباتی ارتباطی و هیجانی در بیماران مبتلا به اختلال شخصیت مرزی انجام‌گرفت.

روش: روش پژوهش به‌صورت کیفی و با رویکرد پدیدارشناسی براساس روش کلایزی است. جامعه آماری آن افراد دارای بیماری اختلال شخصیت مرزی مراجعه‌کننده به بیمارستان‌های شهر همدان می‌باشند که به‌صورت هدفمند 12 نفر از آن‌ها انتخاب و براساس اشباع نظری محقق مورد مصاحبه قرار گرفتند.

یافتهها: در این پژوهش نتایج حاصل از تحلیل مصاحبه‌ها نشان داد که تجربه زیسته شرکت‌کنندگان در قالب چهار مضمون اصلی شامل «کنترل هیجانات»، «بی‌ثباتی خلقی»، «تکانشگری» و «کیفیت روابط بیماران» و ده مضمون فرعی «مهارت تنظیم هیجان»، «ناتوانی در توصیف کلامی هیجان‌های شخصی»، «آشفتگی در کنش‌های عاطفی و شناختی»، «عقاید پارانوییدی»، «نوسانات شدید رفتاری»، «تکانشگری شناختی»، «تکانشگری حرکتی»، «ارتباط بین‌فردی»، «ارتباط با خود»، «ارتباط عاطفی» نمود یافته است.

نتیجهگیری: این مضامین ابعاد مختلف مشکلات بی ثباتی ارتباطی و هیجانی بیماران را در زمینه اختلال شخصیت مرزی آشکار ساختند. یافته‌ها نشان‌دهنده‌ی نیاز به توجه ویژه به تنظیم هیجان، بهبود کیفیت روابط بین‌فردی و مدیریت تکانشگری در مداخلات درمانی این گروه از بیماران است.

Understanding Communicative and Emotional Instability in Patients with Borderline Personality Disorder (A Qualitative Study)

Introduction: Borderline personality disorder is one of the most common, debilitating, and fatal mental conditions that is associated with serious costs and consequences. The present study was conducted with the aim of identifying and understanding communicative and emotional instability in patients with borderline personality disorder.

Methods: The research method is qualitative and with a phenomenological approach based on the Collaizi method. The statistical population consisted of people with borderline personality disorder who were referred to hospitals in Hamedan, of whom 12 were purposefully selected and interviewed based on the researcher's theoretical saturation.

Results: The results of the analysis of the interviews were 4 main themes: "emotional control", "mood instability", "impulsivity", and "quality of patient relationships" and 10 sub-themes: "emotional regulation skills", "inability to verbally describe personal emotions", "disturbance in emotional and cognitive actions", "paranoid beliefs", "severe behavioral fluctuations", "cognitive impulsivity", "motor impulsivity", "interpersonal communication", "communication with oneself", and "emotional communication”.

Conclusion: These themes revealed various aspects of interpersonal and emotional instability in patients within the context of borderline personality disorder. The findings emphasize the need for particular attention to emotion regulation, improving the quality of interpersonal relationships, and managing impulsivity in therapeutic interventions for this patient group.
